Fees/Payments:

Charges for office visits reflect the individual nature of each patient's problem. A single fee is submitted for each surgical procedure or fracture care, which is comparable to the usual and customary charges allowed by insurance companies in our area. This fee usually includes up to three months of follow-up care, but excludes charges for x-ray or cast changes. Each patient is responsible for the payment of his or her account. If you have no insurance, you will be expected to make payment at the time of service. Our staff can discuss payment arrangements with you. We accept Visa, MasterCard, American Express, Personal Check, Medicare, and a vast amount of Insurance Companies. You may also contact our office to verify your specific plan.

Healthcare Plans:

We accept assignment from Medicare as well as participate in managed care insurance programs. The staff will be happy to review your insurance coverage and submit your insurance claims. Be sure to check with your insurance company to make sure the doctor you see is on your plan. You should make sure you know your individual insurance coverage, as every plan is different. Some plans require the patient to have a referral from their primary care physician. If your particular plan has co-pay, you will be expected to pay that at the time of your appointment.

Procedures Specific To Our Practice:

You will need to go to the admissions area of the hospital three days prior to surgery to pre-register. You will be asked to complete paperwork regarding your health and insurance coverage and may have blood work done. You will likely report to the hospital the day of your surgery. Dr. Duke will meet with you prior to surgery to review the procedure and answer any questions that you may have.
